- by visiting the services listed on our website when you apply to be one of our carers or employees.
This privacy policy document contains how we may use information about our suppliers and other contacts. However, this policy does not apply to our employees or Individuals working for Chosen Care Group.

- Personal medical information including past medical history or any requirements related to the medical condition;
- Care plans;
- A Service User’s care preferences which may include the details of the pets they keep and whether they allow the Carer to wear shoes inside their home;
- Financial credentials which may include credit/debit card numbers;
- Continuous reviews.
Chosen Care Group may also collect information which may include details about your health or ethnic origin. We will seek your consent in writing when we need to process such data as required by the Data Protection Legislation.
We may collect the following information from our clients or next of kin of Service Users:
- Your personal details which may include name, address and contact number along with a photo ID;
- Nature of their relationship with a Service User;
- Financial credentials which may include credit/debit card numbers;
We may collect the following information from applicants to Chosen Care:
- your name and date of birth;
- your home address and contact details which may include your phone number and/or email address;
- your educational and professional qualifications as required by our policy and by the clients, your right to work, visa status, employment status; and
- your past experience of working as a caregiver;
- details about your training courses;
- the type of care you prefer to provide;
- any additional information including your CV; and
- how you heard about Chosen Care Group.
Purposes of processing and use of your personal information
Chosen Care Group may require you to provide or confirm all of the above information or other details so that we can deliver our best services without any compromise and to provide you with a great Website usage experience.

We need to process your personal data for the performance of contracts to which you will be a party and when and if required by a legal or regulatory obligation and for our interests.
Chosen Care Group will only process sensitive or special information which may include details about your health conditions to be able to provide care services.
When you fill and complete the Data Protection Consent Form as linked to the Chosen Care Terms and Conditions, you grant us consent as stated in the Data Protection Legislation for Chosen Care to process special categories of personal data.
