    Annual Leave Request Form






    All grades of staff are required to give 4 weeks written notification of a request for paid annual leave.

    Every effort will be made to ensure that staff can take annual leave at the time they wish. There are occasions, where the requested dates are not approved due to care coverage requirements and staff will be requested to pick alternative dates accordingly. Annual leave period is April to March (or) Jan to Dec(refere your contract) and we do not roll-over the annual leave to the next financial year.

    Chosen Care Group cannot be held responsible for any losses or cancellation fees that staff may incur if they have booked and paid for a holiday prior to receiving approval to take annual leave at the times requested and where such approval is subsequently not given due to care coverage requirements. Unpaid leave may be approved in exceptional circumstances and only with the written approval of the line manager.
